Medication Letter from Nurse Caroline (DOC 29 KB)
This is a letter from our nurse discussing the protocol for administering medication at our school.
Request to Administer Medicine (DOC 31 KB)
This form MUST be filled out in order for your child to have medication administered to them during the school day. Please bring this form (filled out) to the school along with the medication. Do not send this with your child. Medication must be dropped off by an adult in the front office.
Excuse form (PUB 46 KB)
Absent note form for you to send in on why your child was absent.
