You do not need to use this form. You are welcome to verify service to others with letters from the agency or organization. Some BEST instructors, like Chef Harold LeBlanc,maintain their own internal records of service. You may use these verified hours to satisfy the service requirement. The District requirement is 20 hours of service for all graduating seniors. Please be sure to give this information to your counselor for recording on your transcript.
