Required Forms For Program Acceptance

Registration Form:

All children enrolled in the program are required to have a completed and signed Registration Form. A new Registration Form must be completed if your child's schedule changes. It is important to inform Kid View, Inc. about any address or phone number changes so that we may reach you in the event of an emergency.

Health Assessment Form:

The Department of Public Welfare required that all children enrolled in the program have a current Health Assessment. This form allows our teaching staff to identify any special needs that your child has and helps to ensure a happy and secure environment for your child. It is the responsibility of the parent/guardian to alert our teaching staff about any health conditions as well as any food allergies that your child may have.

Emergency Contact and Consent Form:

All children enrolled in the program must have a completed Emergency Contact and Consent Form. This form allows our staff to provide first aid care for your child and gives permission for treatment and transportation by emergency medical personnel if necessary. This form is a requiremnent of the state and insurance regulations do not permit us to enroll your child with the emergency and medical forms properly completed and signed. This form also insures your child's safety by listing individuals that you authorize to pick-up your child from the program.
