Mandated Reporter Abuse or Neglect Hotline - 1-800-635-1522 - Below you will find an attachment which provides an Abuse/Neglect Prep Outline. It will help you determine if you should report and help you prepare the information they will ask for. The forms were designed at Madison Central, but ultimately the preparation for the call and chain of command is almost identical. You will use the number above to make a report, but can also use it to consult whether or not you need to make a report. Callers should expect to wait thru the first set of reporting options until the second set is provided. The first option of the second set is for school officials and teachers.
- After making the report be sure to complete the necessary tracking paperwork for the school administration. A copy of the procedure and forms are located in the file section below.

- Herkimer County Mental Health
- Herkimer County Mobile Crisis Assessment Team (MCAT)

- If you would like to be walked thru the process a School Counselor can help, but ultimately the person with intimate knowledge needs to make the call.
- If the call is taken immediately contact the parents and administration.
- After the student is evaluated here in school they may be encouraged to stay at St. Elizabeth's Hospital in Utica for further evaluation.
