Proof of Residence/Child Custody

We require a completed district application along with (2) proofs of residence for all students prior to enrollment.  All necessary paperwork must be housed in your child's permanent record.  Examples of proof that we accept include deeds or housing contracts, leases or rental agreements, utility bills (light, gas, and water), phone bills, etc.   All proof must be in the legal guardian's name, show a physical address (NO PO BOXES) and be current within the past 30 days.  If you have questions, please don't hesitate to ask.

We also need a copy of any custody papework on file in the office as well.  We recognize these documents are confidential and you can block out any sensitive material, we only need to see who has been named as the legal and physical custodial parent or guardian.
