Registration Requirements IF YOU ARE NEW TO THE SCHOOL SYSTEM, you will need: - Proof of Address (current utility bill and one of the following documents: a valid residential lease or rental agreement, a current property tax statement, property deed, or settlement statement for the home) indicating parent’s name and address. If none of these documents are available in the parent’s name due to living with someone else, a notarized Affidavit of Residency must be completed by both the owner/tenant and the parent. The owner/tenant must provide the documents required for proof of address. The Affidavit of Residency must be completed at the Department of Student Services, Central Registration, and both parties are required to be present.
- Child’s Certified Birth Certificate or other approved documentation
- Child’s Social Security Card or Waiver Form (can be obtained from school)
- Current Georgia Certificate of Immunization (may be obtained by taking current immunization record to Muscogee County Health Department, Martin Army Hospital, if military, or to a Georgia physician)
- Certificate of Ear, Eye, and Dental Examination (may be obtained from Muscogee County Health Department, Martin Army Hospital, if military, or a Georgia physician)
- Withdrawal form from previous school or report card (if during summer months)

**New VMMS 6th Graders must provide an updated Georgia Certificate of Immunization (may be obtained by taking current immunization record to Muscogee County Health Department, Martin Army Hospital, if military, or to a Georgia physician) Veterans Memorial Middle School (VMMS) Registration Form Registration forms are available in printed form at VMMS; they are also available at the link below. A parent's signature is required; therefore, a form cannot be emailed or submitted online. A parent should complete and sign a form for each child and take the form to VMMS on Verification Day or at the time of enrollment (summer hours are 8:00 AM - 4:00 PM). If coming during the summer, please call to ensure the school is open (706-748-3203). Online Form Below -
Print out, complete, sign, and carry to Veterans Memorial Middle School at the time of enrollment or, for 6th graders, during Ranger Camp (July 30 & 31, 9-12pm).
