Senior Pages Form

Complete the form below. If you select self and design your page using InDesign the dimensions need to be 9X12. Also please use Photoshop to ensure that photo resolution is at least 300. If you select preset, please select one of the designs and indicate your choice on the form. Return the form, picture CD, and full payment (in an  envelope) to the front office (ATTN: Ms. Shannon) no later than September 8th. No Exceptions. Thank you!
