The purpose of this website is to provide useful information and website links to facilitate the transition process.

Transition is a life long process that requires a great deal of preparation.  In order to be well informed, it is best to start early.  Students, who are turning 21 years of age during the school year, will graduate from P226M, District 75, New York City Department of Education, in June. Students turning 21 during the summer months may attend Chapter 683 summer school. 

It is important to realize that upon graduation, all New York City Department of Education services will end. There is much to know and do to prepare for entering the world of adult services, so let's begin. 


Adult Service Agency Tours and Fairs - Currently Scheduled

Every year, P226M schedules adult service agency tours for the families and students, who will be transitioning.  These tours are open to any and all families of P226M, who are interested in finding out more about these services.  Please let Irene Kawamura, the Transition Coordinator, know if you are planning to attend.  These tours may vary from year to year, so the earlier you start attending and the more that you do attend, the more familiar you will become with what is available in New York City.  You are heartily encouraged to attend as many as you can.  They usually take place on Fridays, at around 10 AM.  The Transition Coordinator will attempt to attend all tours, but is available for any questions by contacting the school. 

These agencies can be contacted directly through the phone numbers below.  If you are unable to tour the sites on the scheduled dates, you are encouraged to make appointments to do so, when it is more convenient for you.

EVALUATION PROCESS

Agencies that provide day services to adults with developmental disabilities are funded through, what is referred to as, straight Medicaid.  Specific assessments (current psychological, psycho-social, and medical evaluations) are, therefore, required by these agencies before placement can begin. 

Please make appointments for final evaluations, as soon as possible.  They should be done early (July-October) in the student's final year in school.   

  • Psychological Evaluation
    • Less than one (1) year old
    • Includes IQ testing results
    • Includes Adaptive Behavior Scale results
  • Psycho-Social Evaluation
    • Less than one (1) year old
    • Includes disability diagnosis before the age of 21
  • Medical Evaluation
    • Less than one (1) year old
    • Includes PPD (tuberculosis) test results
    • Includes disability diagnosis

Copies of all required paperwork should be submitted to the Transition Coordinator as soon as possible, but by December latest, in preparation for the adult service agency application packet exchange scheduled in mid-January.

Please contact the Transition Coordinator, if you wish to request copies of the most currently available Directory of Family Support Services (Manhattan) and/or Manhattan Directory of Day Programs and Vocational Programs for Adults with Developmental Disabilities.


Getting STRAIGHT Medicaid (COVERAGE TYPE 19)

If your child receives SSI, and is under 18 years of age, he or she automatically qualifies for straight Medicaid.  He or she can also request Medicaid Service Coordination, at this time.  If your child qualifies for straight Medicaid, but you are experiencing difficulty obtaining evaluation services, your Medicaid type may NOT be straight Medicaid (coverage type 19).  You must first, disenroll from the Medicaid type that you have and then, enroll in a straight Medicaid program. 

    1. Call the NYC Medicaid office at 800-505-5678.
    2. Verify your child's Medicaid number.
    3. Request an EXEMPTION form.  It will be mailed to you.
    4. Fill it out and sign (may require both the parent/guardian's and student's signatures).  It must be signed by a doctor, too.
    5. Mail back, or submit at a Medicaid office.

You can also walk into any Medicaid office, request this form or request assitance. Have your child's Medicaid number ready.

Families with children, who do not qualify for Medicaid, currently, due to parent salary restrictions, should be aware that at 18 years of age, your child will qualify for Medicaid on his/her own right, because your salaries are no longer taken into consideration.  Prior to your child's 18th birthday, you should actively begin preparing for the straight Medicaid enrollment process.  It takes some time, so start early. 

For parents/guardians of children, 18 and older, who are not currently experiencing any problems obtaining services, with a Medicaid insurance that is NOT straight Medicaid, it is in the best interest of your child, to obtain straight Medicaid, for future purposes.  Remember, at 18, your child will qualify for straight Medicaid.  Medicaid Service Coordinators can help you with this process, as well. If your child has a Medicaid Service Coordinator, please contact them directly and request assistance.

New York Lawyers for the Public Interest

Please be aware that at 18 years of age, the state recognizes your child as an adult.  Depending upon his/her support needs, you may wish to consider guardianship. Guardianship is NOT automatic.  You must petition for guardianship.  This process can be lengthy and complicated, so please contact Ms. Paola Martinez-Boone, for more information. 

MedicAlert

Interested in obtaining a form of identification for your child?  Consider the benefits of a MedicAlert bracelet or necklace.  Not only will it list important contact information, but also vital medical information, in cases of emergency.  In order to receive timely and appropriate assistance from emergency staff, it is necessary to communicate this information.  Don't assume that your child will be able to communicate this information because he or she is verbal, too.  Whether verbal or severely speech impaired, every person needs a way of letting the right people know how to help them--just in case!
