I will ask students to fill out forms and submit them to me.  I can keep track of who completes the forms,the date, and time.  

The form section  allows the student the convenience of completing an assignment at home and without having to keep track of paper, etc. 

Students, make sure you complete the form by the due date given in class.  You should still record form assignments in your agenda.
