PHYSICAL EDUCATION MEDICAL ASSIGNMENT

BIOGRAPHY QUESTIONS

For your medical assignment, you will need to pick a sports figure – they can be alive or dead - and answer the following questions:

1.      What is your sports figure’s given name?

2.      Is there any other name (nickname) that he is known by?

3.      Describe the type of work that your person is most noted for.

4.      Why did you pick him?

5.      What is/was his date of birth/death?

6.      Where was he born?

7.      What is his current age or age when he died?

8.      In what city did your person go to grades K-12?

9.      Did your person go to college? Where?

10.  Describe any difficulty your person had to overcome or endure in order to pursue his sport.

11.  Identify 2 persons who greatly influenced your person. These may include teachers, mentors, colleagues, family members, coaches, etc.

12.  Explain how each one of the persons mentioned in question #11 influenced your person.

13.  Identify 2 of your person’s most notable achievements.

14.  Find 2 quotations – not from your person – that convey the importance of your person (about your person). List whom you are quoting and their relationship to your person.

15.  Explain why your person should be admired or appreciated.

16.  Has your person contributed financially or with his time to any organizations, colleges, and universities outside of his sport?

17.  Which ones?

18.  What do you think are his reasons for his contributions?

19.  In your research, what 2 (one personal and one professional) interesting things did you find out about your person that you did not know previously?

20.  If you were the master of ceremonies in the Hall of Fame induction for your athlete, what would be your first 10 sentences in your introduction to the guests?

This assignment must be typed. The questions can be answered in paragraph form, or you can just list the question number and the answer. Don’t forget to put your name, your PE period and letter days on the first page. List your sources on the last page. You must have at least 2 sources. Return to your Physical Education teacher on or before the due gate in order to receive full credit.
