There will be times that I will ask you to fill out a form online.  I am the only person who will see your responses.  These forms will not affect your grade in any way.  I will use these forms to alter my lesson instructions.  They are meant to provide me with feedback.  Feel free to add any additional information which you think may be useful to me.  The forms are password protected.  I will supply you with the passwords as needed.
