PHYSICAL EXAM SCHEDULE FOR 2011-2012 SCHOOL YEAR

Dr. Kenneth Sebastianelli (School Physician) will be performing the required 11th grade and sports physicals.  If you cannot be at the school on the exam dates or wish to have your own physician perform the physical exam, please obtain the forms from the link at the bottom of this page or at the Nurse's Office.

11th  GRADE PHYSICAL EXAMS

Physicals will be performed by the school doctor in October.  They are scheduled for Tuesday October 18th from 8:30AM to 2PM.  Students that had physicals performed by their own physician need to have the completed form returned by Tuesday February 14th, 2012 .  Failure to do so by this date will result in the student being excluded from school.

FALL SPORTS PHYSICAL EXAMS

Tuesday July 12th, 2011 from 8:30AM-11:30AM and 1PM-5PM 

WINTER SPORTS PHYSICAL EXAMS

Tuesday October 18th, 2012 from 8:30AM-2PM

SPRING SPORTS PHYSICAL EXAMS

Tuesday February 7th, 8:30AM-1PM  (Snow Date February 14th)

FALL SPORTS PHYSICAL EXAMS FOR THE **2012-2013** SCHOOL YEAR

Tuesday, July 10th, 2012 from 8:30AM-11:30AM and 1PM-5PM

DENTAL EXAM SCHEDULE 2011-2012 SCHOOL YEAR

Dr. Edward Grambo (School Dentist) will perform the required 7th grade dental exams on Monday October 24th from 9AM-12N.  Students that had dental exams performed by their own dentist need to have the completed form returned by Tuesday February 14th, 2012.  Failure to do so by this date will result in the student being excluded from school.
