PHYSICAL EXAM SCHEDULE FOR 2008-2009 SCHOOL YEAR

Dr. Kenneth Sebastianelli(School Physician) will be performing the physicals.  If you cannot be at the school on the exam dates or wish to have your own physician perform the physical exam, please obtain the forms from the link at the bottom of this page or at the Nurse's Office.

11th  GRADE PHYSICAL EXAMS

Physicals were performed by the school doctor in October.  Students that had physicals performed by their own physician need to have the completed form returned by January 9th, 2009.  Failure to do so by this date will result in the student being excluded from school.

SPRING SPORTS PHYSICAL EXAMS

Tuesday February 3rd from 9:30AM to 12PM

DENTAL EXAM SCHEDULE 2008-2009 SCHOOL YEAR

Dental exams were performed by the school dentist in October.  Students that had dental exams performed by their own dentist need to have the completed form returned by January 9th, 2009.  Failure to do so by this date will result in the student being excluded from school.
