REGIONAL CENTER or JCDDRB Services To apply for services from Regional Center or Jefferson County Developmental Disabilities Resource Board (JCDDRB) - (they both provide service coordination) Call 1-800-358-7665 and ask for "intake" After applying, the family will be given a choice between Regional Center or JCDDRB. Missouri State I.D. (with picture) You will need to take their original birth certificate and their SS card. The fee is $11.00. Choose a DMV Branch
