 Sport's Physicals

Spring Sports Physicals will be Tuesday,  February 9th 9:00 a.m. until noon (snow date February 23rd.)

1) If you had a fall physical or a winter physical you need to only complete the PIAA CIPPE Section 5  and return it to Mrs. Gorman. You do not need another physical unless you were injured in your previous sport.

2) If this is your first sport season of the 2009-2010 school year then you must complete the PIAA CIPPE sections 1, 2, 3, and the top of 4. These forms must be fully completed and signed by your parent/guardian and returned to Mrs. Gorman before you receive your physical.

3) If you are receiving your physical from your family doctor they must complete the PIAA CIPPE form as well. No other form will be accepted. All forms can be obtained in the nurse's suite or by clicking on the link below.
