Required Athletic Paperwork

Welcome to Cypress Bay, home of the LIGHTNING!

Before you will be allowed to participate in any preseason conditioning and/or tryouts, you will need to complete the following paperwork THOROUGHLY* and turn in to Mr. Caruso's office (your coach can do this for you):

1) Physical Evaluation; now good for one year from date of physical

2) Parent Consent Form

3) Sports Information Cover Sheet

4) Copy of your insurance form

5) Transportation form completed

*Failure to not turn in ALL paperwork and complete each thoroughly will result in incomplete paperwork and you will remain ineligible to participate in all athletic events, including conditioning and tryouts.

**Please make a copy of all forms for your records in case there is a dispute as to whether your forms are turned in or not.  Thank you.
