Parents of sixth graders please be sure to submit your child's Tdap immunization if your child has turned 11.

Also it is mandatory that seventh graders provide the Health Office with a physical between the dates 9/7/10 or later.

A physical with the school doctor will be provided at the parent's request.

Please remember to come to see us in the Health Officefor any of your health care needs 

All students new to our district will need to provide a physical exam form to the Health Office.
