Letters of Recommendation

Letters of recommendation are an important part of your senior year.  You often need letters of recommendation for your college applications, and almost always need them for scholarship applications.  The attached forms should be used when requesting a recommendation from a teacher or the student advisor.  It will assist the teachers and student advisor if the forms are submitted through e-mail.

PARENTS:  Please fill out the parent form and send it into the student advisor.  The original will stay in your student's file and will be kept confidential.  The information provided will assist the student advisor in filling out recommendation letters.  Thank you for your assistance.

Teacher Recommendation Form (doc file)
Give this completed form to the teacher with at least two weeks notice before your deadline.

Recommendation Form - Student Advisor (doc file)
If you would like a recommendation from the student advisor, please complete this form and turn it in at least two weeks before it is due.

Recommendation Form - Parents (doc file)
Parents, please fill this out and give turn it into the student advisor as soon as possible. Thank you.
